1. Leave the domain registered where it is, and point to the DNS nameservers of your webhosting package.
2. Have the domain registration moved to your webhost.
I tend to treat registration and webhosting as separate entities (this can save you a lot of money if you have multiple domains):

You will need to 'unlock' your domain with heartinternet and get a code from them - ask the new company to take over the domain and give them the code - it usually takes a few days - a third part company that handles domain transfers will also probably be involved as a kind of independant adjudicator...
My advice would be initially to just change the DNS settings, cancel hosting but keep registration with your current providers (especially if this is a .co.uk domain). I have had a look at the intended destination of your site and I have to say I was surprised and intrigued by eu5.org - however I very much doubt they will regsiter your domain for free, so I think option 1 is your only real option.
